The proposed metrics include:
1. Community health worker workflow
2. Average interactions per patient per month
3. Methods of contract with patients
4. Type and number of contacts per community health worker per month

Additional details

These metrics have been internally piloted and validated. They plan to tie these metrics to relevant clinical outcomes including ED. visits, primary care physician visits, primary care no show rate, and total medical expenditure.
